With the passing of time, our face tends to lose volume and this together with loss of skin elasticity and actinic damage results in sagging, droopiness and hollowness. The ageing process varies from person to person and the degree of volume loss is also variable. The ageing process results in loss of collagen and elastin, the building blocks of our skin’s architecture. Types of fillers

The basic building blocks of our skin are elastin and collagen. Elastin is an elastic fiber found just underneath skin and collagen is at a deeper level and is found in many different varieties and basically together they give our skin resilience, texture and the feeling of health. Hyaluronic acid is an integral support molecule for collagen and elastin to help keep our skin looking young. As we age and decreased hydration occurs, we lose vital hyaluronic acid, collagen and elastin. Environmental exposures also contribute to the loss of these important skin structures, such as sun, smoking and pollution. As a result of this process, we develop wrinkles, lines and hollows.

Dermal fillers can be divided into two categories: Temporary and Semi-Permanent.Hyaluronic Acid or HA fillers that we use in our practice are temporary fillers.

Injecting hyaluronic acid generates volume in the face, giving it a healthy, youthful appearance. When injected, the HA molecules draw in water and cause the tissue underneath to swell. They’re made of biodegradable, non-animal HA and, unlike collagen, pose no risk of allergic reaction or being rejected by the body. The other wonderful thing about HA fillers is that if you don’t like the effect, we can inject an enzyme called hyaluronidase, again something found naturally in the body, that will dissolve it completely or partially away.

HA fillers include Restylane, Perlane, Juvederm and Voluma. They differ in their molecular size, water absorption and longevity as well as the plane of depth of injections.

It is these characteristics, together with patients’ unique personal features that help us decide which filler to use in different areas. For example, the tear-trough or under-eye area, with it’s thin and translucent skin warrants the use of a product with a smaller molecular size and less water absorption, as opposed to the midface and cheeks where a more robust molecular size filler would be a more viable option.

What areas can fillers address?

When deciding where to inject filler, there are a few features that need to be first carefully considered by your surgeon. Skin is examined for texture and elasticity, tone and laxity. Next the areas of volume loss such as cheek and under eye area are carefully examined. Areas of sag such as jowls are also looked at. Also scars, surgical incisions and genetic bony structure also need to be considered.

Once the face is well-studied, the areas of concern can be discussed at length and a game-plan created. Most frequent areas of concern include under-eye “dark circles”, tear trough deformity, midface sag, jowls, nasolabial folds, receding lips, laugh lines, and scars.
